Cancellation form

We’re sorry to hear that you, your son, or your daughter would like to end the membership at FC VVC. We hope you enjoyed playing football at our club and thank you for your involvement.

To cancel the membership, please fill out the form below. The membership administration will then process the cancellation.

Important information

A cancellation must be received by the membership administration by May 31st at the latest to end the membership at the end of the season.

Filling out this form is the only official way to cancel the membership. Notifying a leader, trainer, or captain is not valid.

A cancellation becomes final when the following conditions are met:

  • The cancellation is received by May 31st at the latest.
  • The match shirt provided on loan has been returned to the team.
  • Any clothing kit and/or bag provided on loan has been returned to the team.
  • The full membership fee has been paid. In the case of installment payments, this means the final installment must also have been received by FC VVC.


If all conditions are not met, we are forced to place a block on the KNVB player pass. This makes any transfer to another club impossible. As soon as all outstanding matters are settled, this block will be removed immediately.
